When you come for your appointment Remember to get the following :

• Medicare or the veteran affairs card.
• Referral letter from your GP, family physician or other doctor.
• Have your insurance information with you.
• Bring copies of records and x-rays, MRIs, CT scans, etc. from clinics outside.

You may be able to avoid repeating a study if your information is here. You may also arrange to have the records sent here prior to your appointment.

To cancel an appointment:

Please call on 02 9745 2587 during office hours and let us know at least 24 hours in advance so we can give that time to another patient.
